This position is responsible for providing skilled nursing and home health aide services to patients, including terminally ill patients, in their place of residence.
Ensures, under the direction of a Registered Nurse, the efficient delivery of skilled nursing and home health aide services, hereinafter referred to as services while at a minimum adhering to state and federal laws, state and federal rules, accrediting body standards, accepted discipline practices, ethical standards, agency policies and procedures, the agencys mission and values, the agencys code of conduct, and business decorum.
Conducts visits
- Accurately completes weekly schedule and conducts visits ensuring compliance with established visit frequencies.
- Identifies self as a representative of the agency to patients/clients and their families at the start of visits, as necessary.
- Ensures the start and completion of visits are independently documented using telephony or other prescribed method.
- Provides assistance to agency Case Managers pursuant to the established patient/client plan of care.
- Ensures patients/clients overall appearance reflects good personal hygiene.
- Ensures patients/clients home environment is safe and orderly.
- Ensures the accurate and efficient procurement and utilization of equipment and medical supplies.
- Works to maximize patient/client outcomes.
- Ensures report has been received from the Case Manager prior to the visit and calls report to the Case Managing after the visit.
- Exemplifies and promotes positive, professional, and effective communication between and among co-workers.
- Ensures the accurate completion of all visit documents in a timely manner.
- Ensures the accurate documentation of visits in a timely manner.
Effectively represents the agency in a professional manner.
Communicates significant events and other data necessary for timely and accurate decision-making to the appropriate individuals.
