REQUIREMENTS/QUALIFICATIONS:

  • Sensitivity to aging, disabled and diverse populations
  • Experience working with community services network
  • Experience in program development
  • Strong interest in working as a member of a staff team
  • Bilingual a plus but not required. 
  • Degree in relevant field preferred. Education can be substituted for  three years of relevant experience.
  • Ability to earn the confidence and develop sound relationships with the broadest spectrum of residents, while maintaining professional work ethic.
  • Demonstrated integrity on a personal and professional level.
  • Good organizational and time management skills.
  • Basic familiarity with word processing software.
  • Ability to communicate effectively both verbally and in writing.
  • Demonstrated ability to advocate, organize, problem-solve and provide results for the elderly and disabled residents.
  • Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly disabled in the area.

SPECIFIC RESPONSIBILITIES:

  • Welcome new residents (and establish contact with existing residents). Provide clarification, information about the resident services programs.
  • Assist and support residents with accessing local services and resources.
  • Provide individual case management for residents including: information and referral, assessment, coordination of community services, crisis intervention and other support as appropriate. 
  • Help set a tone of inclusion and non-discrimination in the resident community.
  • Identify, assess, select, develop and maintain referral partnership relationships with local service resource agencies that effectively assist residents to achieve their opportunities and objectives in life.
  • Provide supportive linkages between residents and referral agency staff when residents or agencies request assistance. Document these efforts.
  • Refer lease violations to the Property Manager and work with the property management team when a resident is identified as being in jeopardy of eviction.
  • Offer linkages and referral support to the resident to positively and quickly rectify the situation. Document these efforts.
  • Maintaining emergency information cards and case file on all residents.  Document all service contacts with residents and their service providers.  Keep all information confidential in a locked drawer. 
  •  Assess and maintain regular contract with frail or at-risk individuals.  Communicate with families, and health and home care providers serving residents.
  • Coordinating educational and health programming in the community.
  • Accessing resources to benefit residents by developing positive working relationships with community agencies, health/home care providers, and senior organizations arranging for participation in service programs such as health clinics, food programs, and transportation services.  Provide link between community service/volunteer programs and residents.
  • Work closely with Property Manager and Administrative Assistant with annual recertification and waitlist updates. Orient new residents to the building, apartment, social opportunities and resident programs; within a month of move-in and orientation, meet again with new residents to answer questions and review house rules, etc. where appropriate. 
  • Other duties as assigned.
