Overview

The Director of Social Services is responsible for the direct provision of social services to residents in Skilled Nursing and their families. This role involves collaboration with an interdisciplinary team to assess residents' needs and requirements.


Responsibilities

  • Offers concise counseling to residents and their families concerning significant life events, such as losses and transitions. Coordinates ancillary services, which encompass, but are not limited to, psychiatric, podiatric, dental, and ophthalmic services for residents in Skilled Nursing, focusing on the identification of needs, referrals, and follow-up care.
  • As a member of the interdisciplinary team, is responsible for conducting psychosocial assessments and providing follow-up care for residents in the Skilled Nursing Facility, in accordance with Title XXII and MDS guidelines for developing care plans.
  • Monitoring the resident's behavior modification plan conducted in accordance with resident assessments to ensure compliance with facility, state, and federal regulations.
  • Part of the interdisciplinary meetings dedicated to discussing the transitions of residents between various levels of care, discharge planning, and other evolving needs. This includes the coordination of resident and family care conferences.
  • The role involves advocating for residents and their rights, particularly in matters related to theft, loss, concerns, and grievances. It also encompasses providing information, assistance, or referrals for Social Services as needed for residents and their families, including guidance on financial eligibility processes and the necessary documentation.
  • Collaboration with the Activities department, Chaplaincy, and other departments is essential to promote the psychosocial well-being of residents.
  • Conducts in-service training and is actively involved in the orientation and palliative care programs for staff at Mercy Retirement & Care Center.
  • Serves as the Department Manager and participates in relevant administrative meetings and required committees.
  • Involves collaboration with the administration regarding on-call coverage for the Care Center.
  • Performs additional responsibilities as assigned.


Qualifications – Position Requirements

  • At least 18 years of age.
  • Cleared Criminal Record clearance through the Department of Social Services prior to presence in the community.
  • Health Screening and Tuberculosis (TB) test results no older than 6 months.
  • Current First Aid Training and Certifications as per Title 22.
  • State licensure preferred (CSW, ACSW, LSW, LCSW)
  • A master's degree a plus; specifically, a Master's degree in Social Work (MSW), a Master of Arts (MA) degree in Counseling, or a related field is acceptable.
  • Bachelor's degree in Social Work or a human services field, which may include sociology, special education, rehabilitation counseling, or psychology, is also acceptable.
  • A minimum of two years of experience in providing services to older adults is required, with a preference for experience in skilled nursing facilities or retirement housing environments.

Skills and Attributes

  • A comprehensive understanding of the needs and challenges associated with older adults and the implications of aging within a social context is essential.
  • The ability to identify need, develop, and oversee programs and systems simultaneously is necessary.
  • An understanding of the healthcare environment and the medical considerations pertinent to older adults is essential.
  • Familiarity with dementia is highly desirable.
  • Exceptional written and verbal communication skills is required.
  • The ability to engage professionally with residents, families, colleagues, and staff in a diverse cultural setting is important.
  • Experience in social work, particularly in direct interaction with patients and their families in a skilled nursing facility, is preferred.
  • Proficient computer skills, including data entry in electronic healthcare records, as well as familiarity with Outlook and Office Suite.
  • Must be a self-motivated and independent individual capable of organizing their workload to ensure all tasks are completed effectively.
  • An enthusiastic, initiative-taking approach in a leadership capacity for the benefit of the entire Mercy Community and Elder Care Alliance is expected.
