REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES
This position reports to Director, ProMedica Pharmacy Medication Management.
POSITION SUMMARY
Under the direction of the Director of ProMedica Pharmacy Medication Management, performs ambulatory care clinical pharmacist functions necessary for the daily operations. The clinical pharmacist must be willing to cover at any ProMedica Pharmacy Medication Management site at any time as deemed appropriate.
ACCOUNTABILITIES
REQUIRED QUALIFICATIONS
Education: Must be a graduate of an Accredited School of Pharmacy (ACPE) and have a Doctorate of Pharmacy (PharmD).
Skills: Must be able to understand directions, communicate and respond to inquiries effectively and timely - requires effective interpersonal skills. Must function as an integral member of the team. Must be able to input and retrieve information from the computer.
Years of Experience: Must have completed a PGY-1 (Post Graduate Year 1) pharmacy residency (for all new hires) or at least two to three years of hospital pharmacy experience (for any previous hires).
License: Must be an actively registered pharmacist (RPh) licensed in state of practice – primary source online verification required directly from State of Ohio Licensing or State of Michigan Licensing database. Must obtain other state license within 1 year of hire.
Certification: Must have a certificate of registration and a current identification card in state of practice. Must maintain Basic Life Support certification.

WORKING CONDITIONS
Personal Protective Equipment: Must wear appropriate PPE. Must wear gloves and suggested to use protective eye wear when performing point-of-care testing.
Physical Demands: Must be able to lift light materials. Non-material handling of continuous sitting and reaching; occasional standing, squatting, kneeling, walking, and stairs. Must be able to move about and between workstations. Must be able to visually tolerate exposure to blood.
